I will be entering the $250 cash Double Down Calcutta. I understand that payment is due at weigh in and failure to pay the $250 cash payment in full with result in my disqualification from the tournament. 100% payback in same structure as tournament payouts.
All teams are required to have 2 members of the EPSFA in good standings for the Fall Trout Challenge. Please list the first and last name of each member below. The name must match the information submitted at the time you did your membership enrollment. If you do not currently have 2 members of the EPSFA on your team, enter the names below that will become members and visit www.epsfa.com/membership to sign up prior to October 18th, 2024.

I acknowledge that I have read and understand the rules and regulations of the EPSFA Fall Trout Challenge and agree that myself and all persons on the boat during the competition will abide by all listed rules of the EPSFA and fishing laws of the PA Fish and Boat Commission. Failure to do so may result in disqualification from the event and possibly all future events held by the EPSFA.
I understand that I am required to complete a tournament disclaimer the morning of the tournament. The form is located on page 5 of the rules. I am required and will send a photographic copy of the completed form to Paul O at (814) 566-4995 before 7:00AM.
